📋 About your medication
Bliv offers both compounded GLP-1 medications prepared by licensed compounding pharmacies, and access to branded GLP-1 medications manufactured by Novo Nordisk (Ozempic®, Wegovy®) and Eli Lilly (Mounjaro®, Zepbound®). Your prescription will specify which type you have been prescribed. Compounded medications are not FDA-approved finished drug products, though they are prepared under federal standards by licensed pharmacies. Branded medications are FDA-approved.
How eligibility is determined
Bliv does not auto-approve patients for GLP-1 treatment. Every patient completes an online intake form and a clinical review conducted by a licensed Bliv provider. Your provider evaluates your health history, current medications, and treatment goals before making a prescribing decision. Submitting an intake form does not guarantee a prescription.
General eligibility criteria
GLP-1 medications are indicated for chronic weight management. Bliv providers generally consider treatment appropriate for patients who meet one or more of the following:
- BMI of 30 or higher (obesity)
- BMI of 27 or higher (overweight) with at least one weight-related health condition, such as type 2 diabetes, high blood pressure, high cholesterol, or obstructive sleep apnea
- A clinical need for metabolic support as determined by your provider during intake review
These are general guidelines, not automatic thresholds. Your provider may approve or decline treatment based on the full picture of your health — not BMI alone.
Conditions that typically disqualify
- Personal or family history of medullary thyroid carcinoma (MTC)
- Multiple endocrine neoplasia syndrome type 2 (MEN 2)
- Current or recent history of pancreatitis
- Pregnancy or actively trying to conceive
- Current breastfeeding
- Age under 18
- Severe or unstable eating disorder (active anorexia or bulimia)
Conditions requiring provider review
- Type 1 diabetes
- Severe kidney or liver disease
- History of gallbladder disease
- Currently taking insulin or sulfonylureas
- Heart failure or recent cardiovascular event
- History of gastroparesis or severe GI motility disorder
What happens during intake
1. Complete the intake form — answer questions about your medical history, current medications, height, weight, and treatment goals.
2. Provider clinical review — a licensed Bliv provider reviews your intake.
3. Prescription decision — if appropriate, your prescription is sent to the pharmacy.
4. Ongoing review — your provider reviews your treatment at each reorder.
What if I am not approved?
If your provider declines to prescribe GLP-1 therapy, you will not be charged for a consultation or prescription.
